The nurses would like to repeal this form, as they currently use form JHCD-E(1), "Administration of Medications to Learners Consent Form." Right now, families are required to have their physicians complete two forms for their child to self-administer asthma medication. However, form JHCD-E(1) already permits parents to give consent for their child to self-administer. |
